Clinical Support Team

Jo - Physician's Associate

Jo is our Physician's Associate and has a keen interest in diabetes and mental health. She assists the GP's, assess, diagnose and treat.

 

Charlotte  - Advanced Nurse Practitioner

Charlotte is our Advanced Nurse Practitioner. Charlotte is able to triage, assess, diagnose and treat. She is able to prescribe medication, and is committed to the advanced nursing provision and supporting the activities of the GPs.  Charlotte is also a registered Queens Nurse.

 

Sarah  - Advanced Physiotherapist

Sarah joined the health centre in 2021 and is our Advanced Physiotherapist.  She leads on our Musculoskeletal and physiotherapy needs and is available across all sites, working two days per week at each.

 

Josephine - Community Clinical Practitioner       

Josie joined the surgeries in 2022 from the local Community Team. Josie supports the clinical team by offering follow-up home visits and supporting newly discharged patients who are housebound. She is the lead for Learning Disability care reviews. Josie also supports the Practice Nursing Team providing dressing clinics and Diabetes care. 

 

Nia - Pharmacy Technician

Nia is our Pharmacy Technician.

The pharmacy technician role includes processing medication queries from patients and staff, advising on sourcing and safe management of medicines as appropriate, assisting in submission of claims for certain medication, interacting with patients by participating in medicine reviews and liaising with local pharmacies and healthcare providers.

 

Leah Connor - Occupational Therapist [Local Health Board]

Leah is an Occupational Therapist employed by the health board.  She offers an Occupational Therapy service within Plas Menai Health Centre, Gyffin and Llys Meddyg.

Occupational therapists enable people living with a range of health problems and chronic conditions to overcome barriers so they can participate in everyday life, and improve their health and wellbeing.

Mel - Practice Nurse

Mel is our practice nurse. Mel oversees the daily running of the treatment room.
